The forms are for the seasonal year beginning January 2014. The state verification officer is Nancy Greenley and the new insurer is K&K.

Note Please - this is for paid up members who have purchased ISA insurance for the 2014 calendar year. This insurance is purchased from K&K by the ISA and is then purchase individually by players playing CICS or who are members of ISA. This insurance is not purchased by Ames Hurricanes and the club has no control over claims.
